Condition Guide

THE 6 LOWER QUARTER CONDITIONS

Click any condition below to see what causes it and how physical therapy helps.

Hip & PelvisGreater Trochanteric Pain Syndrome (GTPS)

Hip Bursitis

A painful irritation and inflammation of the fluid-filled bursa overlying the outside of your hip joint, creating sharp, throbbing, or aching pain.

What Contributes to the Cause?

  • Repetitive friction from tight iliotibial (IT) band sliding over the greater trochanter
  • Gluteal tendon weakness or micro-tearing (gluteus medius/minimus insufficiency)
  • Pelvic drop and asymmetric gait mechanics during walking, stair climbing, or running
  • Prolonged compression from side-sleeping on firm mattresses or cross-legged sitting
  • Leg-length discrepancies or foot overpronation altering pelvic alignment

How Physical Therapy Can Help

Gluteal Tendon Loading & Hip Abductor Strengthening

Progressive isometric-to-isotonic resistance training targeting gluteus medius/maximus to absorb hip forces and prevent pelvic drop.

Pelvic Alignment & Gait Retraining

Correcting 'Trendelenburg' gait patterns, restoring hip hinge mechanics, and optimizing foot strike mechanics.

Targeted Soft Tissue & IT Band Mobilization

Relieving excessive lateral myofascial tension without aggravating the sensitive inflamed trochanteric bursa.

Sleep & Ergonomic Modification Protocol

Personalized sleep positioning (pillow-between-knees strategy) and sitting biomechanics to eliminate mechanical compression.

Proactive At-Home Tip: Place a supportive pillow between your knees when side-sleeping to keep your top hip in neutral alignment and decrease bursa compression.
